Subcontractor Pre-qualification Form

Subcontractor pre-qualification is an integral part of our StructureSource program and a prerequisite for working with Structure Home. To prequalify, be sure to carefully read our standard subcontractor, vendor, and supplier information sheet.

Next, complete the pre-qualification questionnaire below to help us better understand your operational capabilities, performance record, and follow through. 

Along with the questionnaire, please submit a minimum of three builder and vendor references. Following verification of the information and references provided, we will update our estimating database for consideration on bidding future Structure Home projects.

    1) Please list a minimum of three builder references. Please include contact name and title, company name, phone number, email, fax, company address, brief description about the project(s) you worked on for the builder.

    2) Please list a minimum of three vendor references. Please include contact name and title, company name, phone number, email, fax, company address, how long you have been working with the vendor and if you still work with this vendor.
